The Safe Schools and Healthy Students Program provides referral assistance, case management support, and/or wraparound service coordination to families of students who are at clear risk for violence or self-harm or other self-destructive behaviors that place them at risk for school and life failure. Referrals to Family Support Advocates (FSAs) come from a variety of sources: School Teams, Youth Service Teams, Service Integration Teams, or through the Threat Assessment or Suicide Risk Assessment Process. In addition to supporting families, FSAs provide the following services to schools and students:
- Mentoring Support, individual skill-building instruction, and monitoring of challenging and/or troubled youth in school settings
- Small group instruction in school settings for at-risk youth on such topics as social skills, school achievement/motivation, relational aggression, grief and loss, and anger management
- Collaboration with school counselors, teachers, and administrators in support of specific student goals (e.g. increased attendance, achievement, behavior change)
- Consultations and in-services to school staff regarding a variety of prevention topics
Referrals
For school counselors and administrators wishing to make a referral for FSA services, click on the Adobe PDF versions of either the regular referral form or the brief service request form. Each form may be filled out electronically, or printed and completed by hand. Consult with your FSA to determine which form to complete. Once you have completed the form, return it to your FSA.
