Special Education Forms (2008/2009)

The 2008/2009 Special Ed Forms provided on this page are based on ODE forms, with the addition of a few WESD-specific forms. This is the same version as the 2007/2008 forms; there have been no changes.

Note that these forms can be used on either a Macintosh or Windows-based computer, and do not require that you have FileMaker Pro installed on your computer.

It is recommended that users access the ODE website to download translated state forms as needed (see link below).

Instructions for Downloading the Forms

1. Click on the button (Download WESD Special Ed Forms for Macintosh or Download WESD Special Ed Forms for Windows) located at the bottom of this section
2. You will receive a File Download Dialog Box (If you don’t receive this dialog box, Windows users should right-click on the Download Special Ed Forms button, and Macintosh users should click and hold on the Download Special Ed Forms button.)
3. Make sure that "Save this file to disk" is checked
4. Click OK
5. Choose the location where you would like to save your file (on your Desktop, for example)
6. Click Save
7. When the dialog box “download complete” appears, click close
8. Unzip your files before proceeding.
9. 9. Once your files have been unzipped, open the SSForms_08 Folder and double-click the "SSForms_08" icon (for Mac) or "SSFORMS_08" application icon (for Windows) to start the program.
10. The password is ‘open’
11. If you want to import your data from the SSForms_05 version, open SSForms_08 and click the ‘Resources and Support’ link from the Main Menu. Directions are provided from there.
