Referrals
How do children join the Special Education programs offered by Willamette ESD? They rely upon you to refer them to us! Whether you are a family member, medical provider, a preschool staff member, a friend, or a staff member of a community agency, you can refer a child or student to be evaluated.
The process to follow depends on the age of the child or student.
If a child is between birth and five years of age, you can find more information about the Early Intervention / Early Childhood Special Education referral process here.
If a student is eligible to attend school from Kindergarten through Grade 12, then the child's school will begin the referral process. If you are a school district employee and are looking for forms that you need, the School Age Referral Forms may be found on our District Forms page. Once the form is completed, you may fax it to 503-540-2979.
