What is Title VI?
The Title VI Indian Education Program is a federally funded program that provides students of documented American Indian / Alaska Native ancestry with supplementary culturally appropriate educational opportunities.
How does your student(s) qualify for Title VI?
Is your student(s) American Indian / Alaska Native? An enrolled or descendant of a State or Federally recognized tribe? If you answered "YES", your student(s) qualify for our Title VI Program.
How to enroll your student?
1. Download the ED506 Form

2. Please fill out the out form completely.
3. For verification, include your member ID number and/or a copy of tribal identification or proof of descendancy.
4. Return your completed ED506 form to your student's school main office
*Please be sure to check only ONE (1) verification box, if more than one box is check, it will void your ED506 form.
*If you have more than one student eligible, you must fill out a separate ED506 form for each child.
